We typically work 3 days a week in the office but offer the flexibility to work remotely. Our office space is accessible by wheelchairs.

The Bromley by Bow Centre (BBBC) delivers a social prescribing service for young people in Tower Hamlets. Following a successful first year, the service has secured funding to further expand its capacity and reach. This is a specialist Link Worker role using a youth work approach to build and develop the service offering tailored support to young people.
Key to the role is one-to-one person-centred support and building trusted relationships. The role acts as a bridge between health services, schools and community organisation, to support and empower young people to access opportunities and take positive steps to improve health and wellbeing outcomes.

Main Responsibilities:

Referrals
• Act as a main point of contact for referrals into BBBC’s youth social prescribing service.
• Work closely with BBBC’s and the borough wide social prescribing team to generate referrals.
• Build on existing referral pathways and proactively develop new relationships with youth services, schools, healthcare providers and

Personalised Support
• Provide tailored non – judgmental and inclusive one-to-one support to young people, focusing on their individual needs, motivations and aspirations focusing on ‘what matters to them’.
• Work collaboratively with young people to co-develop tailored actions plans to identify and take steps towards achieving their goals.
• Take a holistic, youth-centred approach, addressing the social determinants of health that impact young people’s wellbeing, such as mental health, education, employability and social welfare advice.
• Refer and signpost young people to appropriate support services according to their needs.
• Develop, manage and deliver innovative approaches to supporting young people, building on existing service delivery.
• Manage and prioritise a caseload, in accordance with the needs, priorities and any urgent support required.

Group Support
• Co-design and co-deliver workshops and group activities that address key challenges identified by young people such as managing mental health, building self-esteem, employability and social welfare

Advocacy
• Advocate on behalf of young people e.g. liaising with schools, families, or service providers to address barriers and ensure access and engagement to opportunities.
• Encourage young people to become active participants in their communities, fostering a sense of belonging and purpose.

Monitoring and Reporting
• Ensure accurate data entry across internal and external systems where appropriate.
• Ensure the management and collation of data for reporting purposes.
• Utilise data to identify and track unmet needs among young people, using insights to inform and shape the design and delivery of services.
• Support in evaluating the impact of the service.

Main Requirements (for details check the job description and person specification):
• A recognised professional qualification in youth work or extensive experience working with young people.
• Appointments are subject to Family Action receiving a satisfactory disclosure from the Disclosure and Barring Service – Enhanced DBS
