Hackney WellFamily Plus Service Based in City and Hackney
Salary: Starting at £32,750 rising to £35,564 FTE per annum inclusive of Inner London Weighting (Part-time £18,587 – £20,184 per annum inclusive of Inner London Weighting).
Location: Hackney, London
The role offers flexible working, with the option to work remotely part of the week, and some days based at GP practices and the Family Action office in Hackney.
Hours: 21 hours per week (Part-time) - We offer flexible working arrangements - please see below for more details.
Contract: Fixed term to March 2027 with possibility of extension

This is an exciting opportunity for a WellFamily Practitioner to join our WellFamily Plus service in City and Hackney. You will deliver high-quality, short-term interventions within Primary Care settings, providing holistic, person-centred support to individuals and families. Working across GP practices in the borough, you will play a key role in improving health and wellbeing outcomes through personalised, whole-family approaches that create meaningful and lasting change.
Main Responsibilities:
• Delivering high-quality, short-term (up to six sessions) 1:1 intervention for individuals aged 16+ within Primary Care settings, using a whole-family approach.
• Conducting holistic assessments and developing personalised wellbeing plans that address the emotional, social and practical factors impacting health and wellbeing.
• Working collaboratively with GP practices and other health professionals within one designated neighbourhood (out of eight) in City and Hackney to ensure coordinated and integrated support• Promoting co-production by actively engaging service users in shaping their support and identifying meaningful outcomes.
• Maintaining accurate, timely and confidential case records in line with service standards, safeguarding requirements and performance frameworks.
• Building and sustaining positive working relationships with key stakeholders and community partners to improve access and strengthen local pathways of support.

• Experience of delivering high-quality 1:1 support within a health, wellbeing, community or Primary Care setting.
• Knowledge and understanding of holistic assessment, safeguarding, risk management and outcomes-focused practice.
• Strong interpersonal and communication skills, with the ability to build trust and engage individuals from diverse communities.
• Ability to work collaboratively with health professionals and wider partners while managing your own caseload effectively.
• A motivated, proactive and solution-focused approach, with a commitment to equality, diversity and inclusion.
• Appointments are subject to Family Action receiving a satisfactory disclosure from the Disclosure and Barring Service
